Murphy Dental Discount Plan
Benefits:
- No Annual Maximum
- No Deductibles
- No Waiting Periods
- Covers Initial Comprehensive Exam, X-rays, and Healthy Mouth Cleaning (Waived with Sign-Up Fee)
- One 6-Month Check-Up Examination with Healthy Mouth Cleaning
- Periodontal disease patients will receive their 4th periodontal maintenance cleaning at no charge (must be completed within 12 months of sign up date with Murphy Dental Plan)
- With Sign-Up Fee, Coverage Begins Immediately and Continues for 12 Consecutive Months
- Up to 20% or More Discounted Fees on Treatment!
- Coverage for Emergency Visits and X-Rays
Annual Cost:
- Member 12 years and older: $290.00
- Dependents 11 years of age and younger: $180.00

- Upon filling out our enrollment forms and submitting your annual fee, all discounted rates are available immediately!
- The coverage begins the day you enroll and lasts for 12 consecutive months.
Disclosures
- This is a dental discount plan and IS NOT dental insurance.
- This plan cannot be combined with any other dental insurances.
- It is good ONLY at Murphy Dental and cannot be used with any specialists.
- Should there be dental treatment needed following any type of injury where a lawsuit and therefore outside medical, vehicle, trading services, disability or workman’s comp type insurances are involved, this discounted plan cannot be used.
Non-refundable and Non-transferable
No refunds are given if patient chooses not to use their dental plan even if no treatment is required.
All Payments are due at the time services are rendered.
Plan is subject to change with UCR increases.
