Guardian Dental PPO Dental Insurance: Plans We Accept
Most Guardian plans are PPO dental insurance plans and are accepted at Murphy Dental. Note that we unfortunately do not accept Guardian HMO dental insurance – as these plans will not Murphy Dental for services provided.
As mentioned, most plans are PPO and we will always verify each individual’s insurance regardless.
We look at the patient’s insurance history to accurately assess the maximum amount used, deductible met, and frequency limitations (how often you’re eligible for certain types of recurring preventive procedures like exams and cleanings).
Guardian PPO Dental Insurance Plans - What’s Covered?
Preventive Treatments
Guardian dental insurance typically follows the same structure with preventive services such as prophylaxis (non-disease) cleanings, and fluoride which is often covered for children and occasionally for adults. We always check your coverage to confirm. Diagnostic (x-rays) and exams are covered at one hundred percent. There are frequency limitations on panoramic images and bitewing x-rays.
Basic Treatments
Basic services with most of Guardian’s dental plans are at a coinsurance of 80/20 – meaning the insurance will pay 80% and patient portions that you’re responsible for paying is the 20%. Some treatments that fall under this “Basic” category include treatments like periodontal disease therapy, endodontics (root canals), oral surgery (tooth pulled/extraction), and fillings (cavities).
Major Treatments
Major treatment are typically classified at 50% – meaning patients are required for 50% of the cost of the procedure. Treatments considered “Major” would include implants, bridges, crowns, sinus lift, bone grafts, and other surgical and prosthetics treatments.
